The Health Collaborative is a 501 c 3 nonprofit agency whose mission is to improve the health status of the community through collaboration. It is through a large and varied community partner network that we are able to proudly provide information and access to care, we encourage cross collaboration to ensure that our families are connected with the valuable assets that our community has to offer. This Community Health Portal is one of the tools available for immediate information to programs, services, and care.

Bexar County has the 3rd highest teen suicide rate in Texas with teens committing suicide at a younger age, between 10 and 14 years. Research indicates that 80% of these teens saw their pediatrician within four weeks of committing the act.
To address these youth mental health and behavioral issues, the Health Collaborative launched a multi- partner project called Young Minds Matter (YMM). YMM increases awareness of youth mental health issues in Greater San Antonio and develops a referral network to increase access for families to children’s services.
